Florida Statues 232.032 & 402.305, and Florida Administrative Code
Rule 10D-33.088, mandate compulsory immunization as a prerequisite to
school attendance. Updated information regarding NEW compulsory
immunization requirements for school attendance.
- By law, all
students who are entering, attending, or transferring to a public school
in pre-kindergarten through 12th grade must have a completed Florida
Certification of Immunization form DH680, Part A, B, or C, or a
religious exemption form-DH681. An electronically signed copy of a
completed DH680 is acceptable.
- Beginning
with the 2010-2011 school year, students entering, attending or
transferring to the following grades for the first time (Pre-K, 3rd, 4th,
5th, 6th 7th, 8th, and 9th grades) in a Florida
school will be required to have one dose of the
varicella (chicken pox) vaccine.
- Beginning with
the 2010-2011 school year, students entering, attending or transferring
to Kindergarten, 1st
and 2nd grade will be required to have two doses of the
varicella (chicken pox) vaccine.
- The varicella
vaccine is not required if the student has documented history
confirming that he/she has previously been infected with
varicella. Please note, students that have been retained in
the 9th grade must obtain one dose of the varicella vaccine or show
documented history of a previous varicella infection. In
addition, students that have been retained in 2nd grade must obtain
two doses of the varicella vaccine or show
documented history of a previous varicella infection.
- Beginning
with the 2010-2011 school year, students entering, attending,
transferring, or being retained in the 7th grade will be
required to be inoculated with the Tetanus and Diphtheria toxoids and
acellular pertussis (Tdap) vaccine. The Tdap can be
administered to students 10 years of age and above if prior Td booster
was given at least 2 years ago. Students who received a dose of TD
vaccine 2 to 5 years prior to entering 7th grade should be given a
Temporary Medical Exemption and immunized at the appropriate time.
- All children
aged 0-24 months are required to have one dose of the pneumococcal
vaccine.
- Haemophilus
influenza type b (Hib) vaccination is required for public/private
preschool from two months through 59 months of age. The
number of doses required for Hib vaccination varies, depending on the
child’s age and type of vaccine received.
- Two valid
doses of measles vaccination (MMR) are required for students enrolling
in/attending Kindergarten through the 12th grades.
- The Hepatitis
B (Hep-B) vaccine series is required for students enrolling
in/attending pre-Kindergarten through the 12th grades.
- In accordance
with the Florida Plan for School Health Services, all pre-Kindergarten
through 12th grade students must submit documentation
verifying that a Students’ Health Examination was performed within
the 12 month period preceding initial entry into a Florida school.
A completed Student Health Examination (DH or HRS-H Form 3040) form,
including proof of a Tuberculosis Clinical Screening and
appropriate follow up if necessary, should be completed and signed by a
licensed practicing health care provider, and presented to the school
during initial registration.
